How Your Organization Can Lead the Charge in Whole-Person Care to Create a Better Community

The Whole Person Care (WPC) system is a holistic approach to healthcare that looks at vulnerable individuals through a “whole person” lens to address complex needs within their full spectrum. Clinical professionals have pioneered this system to be able to measure a patient’s social determinants of health for low-income communities, including poor nutrition, lack of safe and stable housing, incarceration, and unemployment.

As defined, WPC coordinates health, behavioral health, and social service needs in a patient-centered manner.  There is a continuum of modalities in which organizations are implementing WPC.

In this previously recorded webinar, you’ll hear how WPC has transformed to help break down barriers as well as what impacts the levels of integrated care and WPC have on patient outcomes.  A set of tools will be provided to assist your organization in the pursuit to address the whole needs of each person served.
