Healthcare Ecosystems Are the Future of Whole-Person Care

“The healthcare ecosystems of the future, like other ecosystems, will be centered on the consumer, in this case the patient.” That’s one of the conclusions reached in a recent white paper by McKinsey &Co, and it encapsulates a major emphasis in Behavioral Health and Human Services: whole person care.

With the growth of Certified Community Behavioral Health Clinics (CCBHCs) in particular, and the emphasis on care coordination in general, the concept of whole person care with the client at the center is a major force in Behavioral Health and Human Services.

While there are many aspects of delivering whole person care. This Open Minds article notes, “Person-centered thinking and operations is a tall order made impossible without the proper tech investments.”

McKinsey: Ecosystems Are Built on 3 Layers

According to McKinsey, ecosystems that will enable this type of care are built on three layers:

  • The infrastructure layer, which employs effective data capture, curation, management, storage, and interoperability to create a common data set upon which the ecosystem can operate.
  • The intelligence layer, which converts data elements to consumable and actionable insights.
  • The engagement layer, enabled by the infrastructure and intelligence layers, which effectively curates an end-to-end experience for suppliers who provide services and offerings to clients.

Infrastructure Layer

Core to Streamline’s SmartCare™ platform is the Data Warehouse, which is each organization’s central repository of integrated data from disparate sources. The data warehouse standardizes this raw data so that it can be used consistently throughout the organization. The standardized data–current and historical–is then available for easy, seamless retrieval in workflows, reports, and analyses.

Intelligence Layer

SmartCare™ manages all types of services across all levels of care in a single application, rather than trying to connect disparate systems. This enables the development of unified processes, workflows, and reports that are necessary for truly integrated care management. Combined with the data warehouse and Business Intelligence software, these in turn enable sophisticated analyses and insights that can significantly improve care.

Engagement Layer

SmartCare’s single platform provides a common interface and process flow for all members of the client’s care management team. SmartCare™ provides the ability for treatment team members from different disciplines to work together in an interdisciplinary fashion for the care of a client from beginning to end.

Behavioral Health and Human Services Ecosystems Are Built on EHRs

For Behavioral Health and Human Services organizations, these three layers depend upon a robust, integrated Electronic Health Record (EHR) platform. Streamline’s SmartCare™ is an Enterprise EHR platform that provides a more comprehensive, seamless approach to operations, care, and analytics necessary for whole person care.

Streamline’s SmartCare™ is a next generation Enterprise EHR platform for the Behavioral Health and Human Services market. It manages all types of services across all levels of care in a single application, rather than trying to connect disparate systems. Its unified architecture, interface, processes, workflows, and reports enable truly integrated care management.
